Comments:
An unannounced renewal inspection was conducted today, July 3, 2019 to determine the provider's compliance with family day home standards. The inspection was initiated at 10:30 am and concluded at 1:00 pm. Upon the inspector's arrival four children were in care with one caregiver for a total of seven points. Children were observed playing with toy cars, in the playhouse and watching a children's show. Later children colored, worked in an safety activity book and discussed different safety rules. The children and caregiver completed an art project and read stories. Hand washing was conducted after toileting. Today's lunch consisted of spaghetti, chicken, cantaloup, green beans, strawberries, bread and milk. The main area used for child care and out door play space were inspected along with a walk through of the entire home. The family day home is equipped with supplies and age appropriate materials. The provider had the required information posted. The first aid kit and non-medical emergency supplies were checked and found complete. An evacuation drill was documented on 06/26/2019, an shelter-in-place drill on 01/29/2019 and a lock down drill on 01/29/2019. Children demonstrated the evacuation drill procedures for the inspector. The inspector interacted with the caregiver and children in care.

Four children's records and two caregiver's records were reviewed.
Per the caregiver: Currently there is no medication being administered. No transportation provided.
Violations cited during the licensing period were reviewed for corrections. There was one repeat violation cited. A complete renewal application will need to be submitted to the department prior to the expiration of the license.

Standard #: 22VAC40-111-780-B
Description: Based on review of four children's records, the provider did not obtain the parent's signed acknowledgment of the annual review of the child's emergency contact information for one child's record. Evidence: The most recent review of the emergency contact information in the record of child #2 was dated 05/29/2018, exceeding the annual requirement.

Plan of Correction: Per the provider: I will leave the child's record out so that the parent can review it and sign the acknow;edgment tonight as pick up.

Standard #: 22VAC40-111-100-A
Description: Based on review of four children's records, the provider did not obtain documentation of a physical examination for one child's record prior to the child's attendance or within 30 days after the first day of attendance. Evidence: The record of child #3, enrolled 03/25/2019, did not have documentation of a physical examination on file.

Plan of Correction: Per the provider: I know the parent has taken the child to the doctor. I will ask them to provide a copy of the most recent physical examination.
