Comments:
An unannounced 60 day monitoring inspection was conducted today from 1:45pm to 3:00pm during the conditional licensure period. There were 6 children (20 points) directly supervised by 2 staff. The home, 2 staff records, 5 children?s records, 3 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, listening to stories, and napping. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Standard #: 22VAC40-111-60-B
Description: Based on record review, a child's record was incomplete. Evidence: Child#1's record is missing the address for their second emergency contact.

Plan of Correction: We will ask the parents to provide an address.

Standard #: 22VAC40-111-70-A
Description: Based on review of children's records, the provider does not have documentation that parents were provided in writing required information before the child's first day of attendance. Evidence: Child#2's record is missing the family day home's medication policy and the Liability Insurance Declaration form.

Plan of Correction: We will ask the parents to bring the signed forms.
