Comments:
The licensing inspector conducted an unannounced renewal inspection on Friday, July 12, 2019 between 10:15 am to 12:30 pm. During the inspection, there were observation of seven children in care totaling 14 points with one caregiver present. The licensing inspector reviewed the designated children care areas, bathrooms, kitchen area, outdoor play areas, and emergency supplies at the family day home to determine compliance with licensing standards. During the inspection, the licensing inspector completed observations of the children participating in various activities to include watching television, engaged in free play with peers, transitioning outdoors to play area, participating in play with various outdoor equipment, and transitioning indoors, and completing bathroom breaks. There were five children's, one caregiver's, two assistant caregiver's/volunteer's, and one household member's records reviewed during this inspection. Violations:
Standard #: 22VAC40-111-830-A
Description: Based on review of emergency drills log and interview with provider, the provider did not ensure to practiced emergency evacuation procedures monthly. Evidence: There were no documentation found for a fire drill for the month of May 2019. Interview with provider confirmed that a fire drill for this month was not completed.

Plan of Correction: The provider will ensure all emergency drills are complete monthly.

Standard #: 22VAC40-111-60-B
Description: Based on review of children's record, the provider did not ensure to obtain all required information for children's record. Evidence: Child#2's record was missing address and telephone number for the father's place of employment.

Plan of Correction: The caregiver will obtain required information from parent during pick up today.

Standard #: 22VAC40-111-240-A
Description: Based on observation of play area, the provider did not ensure all areas were maintained in a clean, safe, and operable condition. Evidence: The pink and purple scooter located on the play areas was observed with a cracked plastic part with sharp edges when touched. The children were engaged in active play and this play equipment was accessible to the children during the inspection.

Plan of Correction: The caregiver will remove the broken piece tonight.
