Technical Assistance:

The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing department on June 10, 2019 regarding allegations of supervision and infant feeding routines. The inspection was completed on July 10, 2019. The inspector was on site at 11:09am. Interviews with caregivers were completed, and observations were made in the center. The preponderance of the evidence gathered during the investigation supports part of the allegation; therefore, the complaint is determined to be valid. See the violation notice for more details. Complaint related: Yes
Description: Based on caregiver interview on 07/10/2019, the provider did not ensure that bottles shall not be propped. EVIDENCE: Caregiver #1 stated that she did did prop the bottle of one infant. She stated that the infant could hold her own bottle, but would prop it because it would take the infant a long time to finish the bottle.

Plan of Correction: It was a means of additional support for a child that could hold their own bottle.

Description: Based on caregiver interview on 07/10/2019, the provider did not ensure that Infant formula shall be prepared according to the manufacturer's or physician's instructions. EVIDENCE: 1. Caregiver #1 stated that when an infant wasn't feeling well she has mixed baby food, such as rice cereal , with formula into that infant's bottle. 2. Caregiver #1 stated that the child's parents were aware that food was mixed with the formula, however she did not have a note from the child's doctor.

Plan of Correction: Won't add food to bottle unless have doctor's note.
