Comments:
A Subsidy Health and Safety inspection was conducted on 7/10/19 to review supplemental health and safety requirements. There were 3 children (6 points) present with 1 caregiver. The children were observed playing with toys, reading books, engaging in music and movement activities. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Description: Based on record review, the provider did not maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care. Evidence: Daily attendance was not recorded and maintained for 7/8/19 and 7/9/19.

Plan of Correction: I will maintain daily attendance.

Description: Based on staff interview, the family day home provider did not implement a continuity plan. Evidence: The provider stated that she did not write and implement a continuity plan or the family day home.

Plan of Correction: I will have the plan ready by the end of this week.
