An unannounced, mandated renewal inspection was conducted today. The inspector arrived at 11:30 AM and departed at 1:30 PM. Nine children, totaling twenty-six points were in care with two caregivers. Lunch service and nap time were observed. Three caregiver records and a sample size of nine children's records were reviewed. An exit meeting was conducted with staff 1 prior to closure of the inspection.
Standard #: 22VAC40-111-100-A Description: Based upon review of nine children's records and staff interview, the provider has not ensured that documentation of a physical examination by or under the direction of a physician is obtained prior to a child's attending or within 30 days after attending. Evidence: 1. The record for child 8 indicated that child 8 began attending on 12/12/2018. The record did not include documentation of a physical examination. 2. Staff 1 verified that documentation of a physical examination was not in the record of child 8. Plan of Correction: Staff 1 responded with the following: The parent will be notified today and will be required to bring in documentation of a physical examination.

Evidence: 1. According to the record, child 8 began attending on 12/12/2018. Documentation of verifying the child's age and identity was not in the child's record. 2. Staff 1 verified that the required documentation was not in the record of child 8. Plan of Correction: Staff 1 responded with the following: The parent will be contacted today and will be required to bring in proof of the child's age and identity or local authorities will be notified.
