Comments:
An announced initial inspection for a new family daycare was conducted July 16 2019 between 12:30pm and 2:00pm. There were 4 children in the direct care of the day care provider. Children were observed during nap time. The home is clean, organized, and offers plenty of toys, books, and materials. Areas of non-compliance are listed on the violation notice. Standard #: 22VAC40-111-60-B
Description: Based on record review, 1 of 4 children's records reviewed did not have all of the required information. Evidence: Child # 3 record does not contain documentation of complete emergency contacts including full addresses.

Plan of Correction: I will contact the parent and have the parent bring the information.

Standard #: 22VAC40-111-70-A
Description: Based on record review, 1 of 4 children's records reviewed did not have documentation that parents of a child enrolled had all of the required forms prior to the first date of attendance. Evidence: Child # 2 record does not contain documentation that the parent received the emergency preparedness form.

Plan of Correction: Parent received the form. I will have them sign a new form tonight.
