Technical Assistance:
Assistant caregiver 2 has not yet completed all required annual training hours. Documentation of the remaining 10 hours of required training must be submitted to the Inspector before the license can be processed for renewal.

Comments:
An unannounced full renewal inspection was conducted today. Two assistant caregivers were caring for 9 children, totaling 21 points. The provider was not home during this inspection. It was reported to the Inspector that the provider was scheduled to be out of the home for a brief period of time, lasting less than 2 hours. Children's records, the physical plant, emergency preparedness items, administrative documents, background checks, caregiver records, caregiver training, and children's nutrition were inspected today. The children were observed playing outside, reading books, and preparing for lunch. The children were to be served beef stew with rice and vegetables, apple slices, bread, and milk for lunch today. This inspection was completed between approximately 10:15am and 11:50am. Thank you for meeting with me today. Standard #: 22VAC40-111-800-B
Description: Based on a review of the emergency preparedness plan, it was determined that the provider has not conducted an annual review of this plan. Evidence: The provider last reviewed the emergency plan on 1/23/18.

Plan of Correction: I will review it with my assistants.

Standard #: 22VAC40-111-110-A
Description: Based on a review of children's health records, it was determined that two records were not complete. Evidence: Child record A does not include a correctly completed physical exam report. This child's physical exam report was not documented on the VA Health Form. Child record C does not include a correctly completed physical exam report and immunization record. These health records were not documented on the VA Health Form with the doctor's signature indicating that the child is adequately immunized for her age.

Plan of Correction: Both parents will be asked to provide their child's health records on the correct form.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that one record was not complete. Evidence: Child record A does not include: the parent's work contact information; the doctor's contact information; the address for one emergency contact person; documentation indicating that the provider has viewed the child's original proof of ID.

Plan of Correction: The parents will be asked for the missing information.

Standard #: 22VAC40-111-240-A
Description: Based on observations, it was determined that one piece of equipment has not been maintained as required. Evidence: One crib mattress has torn and needs to be replaced.

Plan of Correction: I will buy a new mattress.
