Comments:
An unannounced monitoring inspection was conducted today. The provider was caring for 6 non-resident children, totaling 13 points. Children's records, provider training, emergency preparedness items, the physical plant, and children's nutrition were inspected today. The children were observed playing with toys, creating with playdoh, and eating lunch from home. This inspection was completed between approximately 12:00pm and 1:00pm. Violations:
Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that two records were not complete. Evidence: 1 - Child record A does not include: documentation of the child's date of enrollment; the parent's medical insurance information. 2 - Child record D does not include: the parent's medical insurance information; the father's place of employment information.

Plan of Correction: The parents will be asked for the missing information.

Standard #: 22VAC40-111-70-A
Description: Based on a review of children's records, it was determined that two parents have not been provided all policy information as required. Evidence: Child record A does not include a signed "Information for Parents" document; a completed "Provisions of the Emergency Plan" document. Child record D does not include a completed "Provisions of the Emergency Plan" document.

Plan of Correction: The parents will be asked to sign the completed forms.

Standard #: 22VAC40-111-250-E
Description: Based on observations, it was determined that the provider has placed a balloon in an area that does not keep it inaccessible to the children under age five. Evidence: The Inspector observed a filled balloon on the floor in the child care area. There were 6 children in care under the age of five.

Plan of Correction: We won't use balloons.

Standard #: 22VAC40-111-320-B
Description: Based on observations, it was determined that one electrical outlet was not covered. Evidence: One electrical outlet on the bottom portion of the wall in the entry way was not covered. The children enter and exit through this entry door.

Plan of Correction: The outlet will be covered.

Standard #: 22VAC40-80-120-E-2
Description: Based on observations, it was determined that the provider has not posted all required administrative information. Evidence: The provider did not post the findings from the most recent inspection.

Plan of Correction: I will post today's Violation Notice.
