Technical Assistance:
Technical assistance was provided regarding sending a copy of pets vaccine record when updated and inspection of entire house is required.

Comments:
An unannounced renewal inspection was conducted today from 9:30am-11:00am. There were 8 children (21 points) supervised by 2 direct caregivers. The home, programming, 2 staff records, 8 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed engaging freely and eating snack. Diapering procedures were also discussed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Standard #: 22VAC40-111-780-A
Description: Based on review of 8 children's records, 1 is does not have documentation of an annual review of emergency contacts. Evidence: Child #2 does not have updated emergency contact information, the last update was 9/18/18.

Plan of Correction: I will have the parents review and update their emergency contacts.

Standard #: 22VAC40-111-100-A
Description: Based on review of 8 children's records, one does not have documentation of physical within 30 days of first day of attendance. Child #4 does not have record of a physical.

Plan of Correction: I will get a copy of the physical.

Standard #: 22VAC40-111-60-B
Description: Based on review of 8 children's records, one record is missing documentation of two emergency contacts. Evidence: Child #1 has documentation of only one emergency contact.

Plan of Correction: I will get a second emergency contact and update the form.

Standard #: 22VAC40-111-280
Description: Based on observation, potentially poisonous substances are accessible to children. Evidence: A bottle of bleach & water was left out on the bathroom sink, a closet was unlocked making laundry soap and paint accessible to children.

Plan of Correction: I will put locks on the doors to make the substances inaccessible.

Description: Based on review of pet records, not all records are up to date. Evidence: Pet #1 does not have documentation of current rabies vaccination.

Plan of Correction: We have a vet appointment on7/21/19, I will send you a copy of the updated vaccination.

Standard #: 22VAC40-111-690-D
Description: Based on interview with provider, the diapering surface is not being cleaned and sanitized according to licensing standards. Evidence: The diaper changing surface is not being sprayed with soap and water, wiped cleaned, sprayed with bleach and water and allowed to air dry after each diaper change.

Plan of Correction: Proper procedures were corrected during the inspection.
