Technical Assistance:
Had a discussion with the provider regarding a child with a severe food allergy and having the child's registration forms updated with a current action to take in case of an emergency. The provider was reminded to include her own children on the daily attendance sheet for accuracy in an emergency.

Comments:
Today an unannounced Renewal inspection took place between 9:40am and 11:35am. There were 7 children present with the provider and 1 assistant for a total of 18 points within the supervision guidelines. The children were observed playing freely with trucks, trains, magnet toys, working their large motor skills by walking on large steps, playing jump rope and eating a morning snack (veggie straws). Diaper changing and hand washing procedures were observed. The provider is not currently MAT trained and there was no medication at the Family Day Home. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Violations:
Standard #: 22VAC40-111-750-A
Description: Based on observation and interview, sunscreen requirements were not met. Evidence: 1. There were two bottles of non-prescription sunscreen observed on top of the kids cubby area. Neither bottle was labeled with a child's name to identify the owner. The provider stated that she couldn't recall which parents provided the sunscreen and that she applies the sunscreen to all the children when they go outside. 2. There is not written permission from any of the children's parents to authorize the provider to apply sunscreen. The provider stated that she applies sunscreen to all the children before going outside.

Plan of Correction: The inspector printed the topical authorization for the provider. I'll have all the parents complete the form.

Standard #: 22VAC40-111-760-A-1
Description: Based on observation and interview, the first aid kit did not contain the minimum supplies. Evidence: The provider did not have antiseptic cleaner, gloves or tweezers available.

Plan of Correction: I will buy a kit at the store that has all the supplies I have to have.

Standard #: 22VAC40-111-120-B
Description: Based on record review, assistant caregiver's records did not contain all required information. Evidence: Caregiver #2's file did not contain a second reference check.

Plan of Correction: I think I might have called some people but I don't think I wrote anything down. I'll see if I can call someone else to document.
