Comments:
The licensing inspector conducted an unannounced monitoring inspection on Tuesday, July 16, 2019 from approximately 11:20am to 2:30pm to determine the provider?s compliance with licensing standards. During the inspection, there were 8 children in care totaling 20 points in the care of two caregivers. Upon arrival, the children were observed going outside and during water play. The licensing inspector observed the children engaged in play with their peers and interacting with the caregivers. The licensing inspector later observed the caregivers and the children in the main area of the family day home as they prepared for lunch and nap time. The licensing inspector interacted with the children when appropriate throughout the inspection. The caregivers were observed having positive interactions with the children in care. The areas for childcare, bathroom, kitchen, indoor, and outdoor play area were inspected. The required postings were reviewed and found to be in compliance. The first aid kit and non-emergency supplies, daily attendance, and fire and shelter-in-place drills were inspected. The menu was posted and today?s lunch consisted of chicken nuggets, hash brown, bread, vegetables, strawberries, and milk. Medication is not administered and transportation is not provided at this time. During this inspection, five children?s records, three caregivers? records, and one household member?s records were reviewed. Standard #: 22VAC40-111-120-B
Description: Based on a review of three caregiver records and interview on 07/16/2019, two records did not contain the required documentation. Evidence: 1) The record for Caregiver #2, hired on 06/28/2019, did not contain documentation of two or more references as to character and reputation as well as competency were checked before employment. The record for Caregiver #2 contained only one reference check. 2) The record for Caregiver #3, hired on 06/28/19, did not contain documentation of two or more references as to character and reputation as well as competency were checked before employment. The record for Caregiver #3 contained only one reference check. 3) During interview, Caregiver #1 confirmed only one reference check was obtained for Caregiver #2 and Caregiver #3 to date.

Plan of Correction: The provider will obtain the second reference for each caregiver and will complete two references before hire for any new hires.

Standard #: 22VAC40-111-60-A
Description: Based on a review of five children's records on 07/16/2019, the provider did not maintain an up-to-date record at the family day home for each enrolled child. Evidence: 1) The record for Child #1 (DOE: 04/10/2017) did not contain the policy number of the child's medical insurance or annual parent written acknowledgement that the emergency contact information had been reviewed and was correct. The most recent acknowledgment of record review for Child #1 was dated 02/08/2018. 2) The record for Child #2 (DOE: 05/16/2018) did not contain annual parent written acknowledgement that the emergency contact information had been reviewed and was correct. The most recent acknowledgement of record review for Child #2 was dated 06/19/2018. 3) The record for Child #3 (DOE: 09/05/2017) did not contain annual parent written acknowledgement that the emergency contact information had been reviewed and was correct. The most recent acknowledgement of record review for Child #3 was dated 09/05/2017. 4) The record for Child #4 (DOE: 01/12/2019) did not contain the policy number of the child's medical insurance, a signed emergency medical authorization, or a signed liability insurance declaration. 5) The record for Child #5 (DOE: 09/19/2018) did not contain the name and policy number of the child's medical insurance.

Plan of Correction: The provider will review each record with each parent by next week to ensure all the required information is on file.

Standard #: 22VAC40-111-180-A
Description: Based on a review of documentation and interview on 07/16/2019, the provider did not maintain a current Report of Tuberculosis (TB) Screening form for one out of three caregivers every two years from the date of the first screening or more frequently as recommended by a physician or the local health department. Evidence: 1) The record for Caregiver #1 contained a TB screening test dated 06/07/2017. 2) During interview, Caregiver #1 confirmed the TB screening for Caregiver #1 expired on 06/07/2019.

Plan of Correction: The provider will schedule an appointment for next week to ensure the TB is updated as needed. The provider will ensure to update the screening every two years.
