Comments:
An unannounced monitoring inspection was conducted with caregivers on July 25, 2019 to determine compliance with the licensing standards. There were eight children present, ranging in age from 10 months to 9 years old, totaling 15 points. The children were waking from rest time, arriving from summer school, playing with age appropriate toys and activities, and eating afternoon snack. Snack served today: graham crackers and apple juice. Six children's files and three caregiver/household files were reviewed - please see Supplemental Summary for missing items. Caregivers completed first aid and CPR on 11/11/2018. Provider does not administer medication - Medication Administration Training is required to give medicine. Evacuation drills have been conducted monthly. The last shelter in place drill was conducted 3/19/2019. Description: Based on review of six children's files, the provider failed to have two emergency contacts for each child in care. Evidence: Child 6 did not have the name, address and telephone number of a second designated person to contact in case of an emergency if the parent cannot be reached.

Plan of Correction: Provider will request this information from the parent.

Standard #: 22VAC40-111-70-A
Description: Based on review of six children's records, the facility failed to have documentation that all written information was provided to parents prior to the child's first day of attendance. Evidence: the VDSS model form titled "Policy on the Administration of Medication" was in each child's file, but the form was not signed by parent of Child 3, Child 4, and Child 5. .

Plan of Correction: Provider will have the parent of the three children sign the "Policy on the Administration of Medication".
