The licensing inspector conducted an unannounced monitoring inspection on Wednesday, July 24, 2019 from 2:50pm to 4:20pm. The children were observed resting on mats. They were later observed playing with toys and eating a small snack. Today, there were three children totaling six points, present with one caregiver. Five children?s records, and one caregivers? record were reviewed during the inspection. Last emergency drill: 07/03/2019 Last shelter-in-place drill: 7/03/2019 (practiced monthly) The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-111-800-B Description: Based on caregiver interview on 07/24/2019, the provider did not ensure that the emergency plan is reviewed at least annually. EVIDENCE: Caregiver #1 stated that the emergency plan was reviewed in March, but wasn't documented. Plan of Correction: Will sign the emergency plan review sheet today.
Standard #: 22VAC40-111-100-A Description: Based on a review of children's records on 07/24/2019, the provider did not ensure that documentation of a physical examination was obtained prior to a child's attendance or within 30 days after the first day of attendance. EVIDENCE: The record of Child #1, who started at the center on 02/25/2019, did not contain documentation of a physical. Plan of Correction: Will tell parent today and give until 08/09/2019 to get to me.
Standard #: 22VAC40-111-60-B Description: Based on a review of children's records on 07/24/2019, the provider did not ensure that each child's record contained the required information. EVIDENCE: 1. The record of Child #1 did not contain the phone number of the place of employment listed for the child's mother and father. 2. The records of Child #2, 3, 4,and 5 did not contain documentation of the review of the child's emergency contact information. Plan of Correction: Will have them fill it out today
