Standard #: 22VAC40-111-750-B
Description: Based on observation of the family day home it was determined that not all topical skin products were kept inaccessible to children. Evidence: One bottle of baby lotion, baby powder, and one container of petroleum jelly was observed on a low shelf of the changing station.

Plan of Correction: The baby lotion, baby powder, and petroleum jelly were made inaccessible during the inspection.

Standard #: 22VAC40-111-170-A
Description: Based on review of one household member record it was determined that not all household member records contained documentation of a TB (tuberculosis) screening prior to coming into contact with children. Evidence: The record for household member A did not have documentation of a TB screening on file.

Plan of Correction: Household member A will obtain a TB screening and submit the results to the provider for their file.

Standard #: 22VAC40-111-480-E
Description: Based on observation of the crib in the family day home it was determined that not all materials used by children were free from hazards. Evidence: A quilted pad observed in a crib was observed to have tears along all 4 sides. Quilted filling was observed in the areas of the tears making the filling loose and accessible, posing a choking hazard.

Plan of Correction: The quilted pad was removed during the inspection and will be thrown away.

Standard #: 22VAC40-111-520-C
Description: Based on observation of the crib in the family day home area it was determined that not all cribs were in compliance with the Consumer Product Safety Commission (CPSC). Evidence: The white metal crib in the family day home had a manufacture date of 07/27/2009. Per the CPSC, cribs with a manufacturer date before 06/28/2011 shall not be used.

Plan of Correction: The provider will replace the current crib with a crib stored in the provider's garage that meets the CPSC standards.

Standard #: 22VAC40-111-530-F
Description: Based on observation of the crib in the family day home it was determined that soft bedding was not excluded for use. Evidence: Child A (Age 2 Months) was observed laying on their back, on a soft quilted pad in the crib.

Plan of Correction: The provider removed the pad during the inspection.
