Comments:
An unannounced Subsidy Health and Safety Inspection (SHSI) was conducted from 11:00 a.m. - 12:30 p.m. There were four children in care with the provider ages 2 months, 21 months, 22 months, and 23 months for a total of 13 points. There are reportedly no children enrolled with diagnosed food allergies. The most recent lockdown drill was conducted 02/15/2019. The provider record was observed for completion of CPR and First Aid Training. Standard #: 22VAC40-665-230-B
Description: Based on interview with the provider it was determined that the provider did not have documentation of completing the Virginia Preservice Training course. Evidence: The provider was not able to provide documentation of completing the Virginia Preservice Training course.

Plan of Correction: The provider will attempt to locate the Virginia Preservice Training certificate. If the provider is unable to obtain documentation that the course has been completed the course will need to be repeated.
