Technical Assistance:
Technical Assistance was offered regarding the menu, high chair and cooling fans.

Comments:
An unannounced renewal inspection was conducted from 10:40am to 12:45pm. There were 5 children (11 points) with two caregivers. The children were observed watching cartoons on TV, playing with legos, blocks, and eating lunch. Lunch served did not match what was listed on the menu. The menu included rice, chicken, fresh fruit and vegetables. 6 children's records, two caregiver and two household member records were reviewed. Emergency drill records and plan was reviewed. Violations:
Standard #: 22VAC40-111-800-B
Description: Based on record review, the provider did not review the emergency preparedness plan was not updated annually. Evidence: The emergency preparedness plan was last updated on 1/1/19.

Plan of Correction: The plan will be updated.

Standard #: 22VAC40-111-900-A
Description: Based on observation, food was served to children under the age of two that was not cut lengthwise. Evidence: Grapes were served to a child under the age of two. The grapes were not cut lengthwise.

Plan of Correction: I will make sure that round food items are cut properly.

Standard #: 22VAC40-111-240-A
Description: Based on observation, equipment outside the house was not maintained in a clean, safe and operable condition. Evidence: 1. The round slide on the playground had loose bolts on one attachment. There was a loose piece on another attachment of the slide that was detached from the slide. 2. The play equipment on the playground including the play structure, play houses and bikes were not maintained in a clean condition.

Plan of Correction: The slides will be fixed. All equipment will be power washed.
