An unannounced monitoring inspection was conducted from 2:30 - 4:00 pm with the provider. There were six children in care, ranging from four years to eight years of age, for a total of eight points. The children were observed napping upon arrival, waking, reading books, and eating snack. Snack served today was goldfish, yogurt and water. Four children's files were reviewed. Two caregiver/household member records were reviewed and in compliance. Required information was posted. The emergency drill log was reviewed and in compliance. The provider is not Medication Administration Training (MAT) certified and cannot administer medications to children in care. Standard #: 22VAC40-111-100-A Description: Based on a review of four children's files, the provider did not obtain documentation that each child has received a physical examination before the child's first attendance or within one month after attendance. Evidence: The files for Child #1 (date of first attendance: 10/6/15) and Child #2 (date of first attendance: 10/10/16 did not have documentation of a physical examination. Plan of Correction: Will ask the parents to provide documentation no later than 08/02/19.
Standard #: 22VAC40-111-60-A Description: Based on a review of four children's files, it was determined that not all files were complete. Evidence: The file for Child #4 (date of first attendance: 7/26/19 ) did not have a work phone number for one parent, the phone number for the physician, the medical insurance policy number, or the name, address and phone number for a second emergency contact. Child #4's file also did not have signed documentation that parents had been given VDSS model forms "Information for Parents", "Policy on the Administration on Medication" or ?Liability Insurance Declaration" in the file. Plan of Correction: Will get missing information from parent.
