Comments:
A unannounced renewal insepction was conducted with the provider and one assistant. 11 Children were in care during the inspection and the age range was eight months to four years old total in points was 26. Children were observed during rest time, and story time. The Provider conducted a escape practice while LI was present. Five children's and three house hold member's records were reviewed. Standard #: 22VAC40-111-830-A
Description: Based on review of the emergency evacuation drills not all were practiced monthly. Evidence: There was no documentation of a escape practice practices of the month of June.

Plan of Correction: I will conduct two practices in August.

Standard #: 22VAC40-111-180-A
Description: Based on review of three house old member's records not all had a current T.B. Evidence: House hold member A's T.B. test is dated 2/18/16.

Plan of Correction: Not available online. Contact Inspector for more information.

Standard #: 22VAC40-111-530-A
Description: Based on observation not all resting children had top and bottom linens. Evidence: Three children were resting on cots without bottom linens and one child did not have a top or bottom linens.

Plan of Correction: I will buy extra linens.
