Technical Assistance:
Please remember to conduct 2 shelter in place drills by December 31, 2019. Discussed following up on Staff 3's central registry check of results are not received within 30 days of submission.

Comments:
An unannounced monitoring inspection was conducted today from 11:00am-12:20pm. There were 12 children (30 points) directly supervised by 3 staff. The home, 3 staff records, 5 children?s records, 1 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, watching a children's program, and finishing lunch consisting of homemade vegetables and rice with yogurt and strawberries. and eating snack. There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-120-B
Description: Based on record review, one of three staff member's records was incomplete. Evidence: The record for Staff 3 did not contain: the staff member's address; verification of age; job title; date of employment; name address and telephone number of a person to contact in the event of an emergency; two references; and documentation of education.

Plan of Correction: I will complete the file.

Standard #: 22VAC40-111-60-B
Description: Based on record review, one of five children's records reviewed during the inspection was incomplete. Evidence: The record for child 5 did not contain: the father's employment address, the child's health insurance policy information, and signed authorization for emergency medical treatment.

Plan of Correction: I will have the parent complete the missing information today.

Standard #: 22VAC40-111-70-A
Description: Based on a review of children's records, one of five children's records reviewed did not contain documentation that parent's were provided required written information about the provider's facility before the child's first day of attendance. Evidence: Child 5's record did not contain documentation that the parents were provided information in writing on the following policies: Operating information including the hours and days of operation, holidays or other times closed, and the telephone number where a message can be left for a caregiver; Schedule of fees and payment plans; Check in and check out procedures; Requirement for the family day home to notify the parent when the child becomes ill and for the parent to arrange to have the child picked up as soon as possible if so requested by the home; Requirement for the parent to inform the family day home within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life-threatening diseases, which must be reported immediately; Requirement for the child to be adequately immunized as required by 22VAC-40-111-90; Requirement for paid caregivers to report suspected child abuse or neglect according to ? 63.2-1509 of the Code of Virginia; Custodial parent's right to be admitted to the family day home any time the child is in care as required by ? 63.2-1813 of the Code of Virginia; General daily schedule that is appropriate for the age of the enrolling child; Policies for the provision of food; Discipline policies including acceptable and unacceptable discipline measures; Presence of a pet or animal in the home; Amount of time per week that an adult assistant or substitute provider instead of the provider is scheduled to care for the child and the name of the adult assistant or substitute provider; Parental notifications required in 22 VAC 40-111-650; Policies for termination of care; and Address of the website of the department, with a note that a copy of this regulation and additional information about the family day home may be obtained from the website, including compliance history that includes information after July 1, 2003.

Plan of Correction: I will provide the information to the parents today.

Standard #: 22VAC40-111-530-G
Description: Based on observation, children under 2 years of age were allowed to use pillows. Evidence: Child 1 was observed in a crib with a large pillow.

Plan of Correction: The pillow is being used to support the chid in a seated position as the child has physical development delays. I wil remove the pillow today.
