Comments:
An unannounced renewal inspection was conducted with the provider beginning at 9:50 AM and ending at 11:45 AM. There were seven children in the care of the provider during the inspection. The children were ages 17 months, 22 months, 2, 2, 3, 3, and 4 years old for a total of 16 points. The children were observed playing with home living toys, playing with play dough, and coloring. Bathroom and hand washing procedures were observed. Seven children's records were reviewed and in compliance. Two household records were reviewed and in compliance. One staff record were reviewed. One pet record was reviewed and in compliance. The emergency preparedness plan and drill log was reviewed and in compliance. The most recent emergency escape drill was documented as 7/17/19. The most recent shelter-in-place drill was documented as 6/ 17/19. There were no medications on site to be reviewed. The provider and assistant have current First Aid and CPR training certification. The provider has a current Medication Administration Training (MAT) certification which is required to administer prescription medication. The first aid kit, flashlight, and weather-band radio were observed and in compliance. Standard #: 22VAC40-111-180-A
Description: Based on a review of documentation, the provider did not obtain documentation of subsequent tuberculosis screening reports of all caregivers. Evidence: Staff A's most recent tb screening was dated 5/12/17. Tb screenings are required to be obtained every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.

Plan of Correction: The provider stated that the staff member has already completed the tb screening and the documentation will be placed in the record once it is received.
