Comments:
An unannounced renewal inspection was completed on July 30, 2019 from approximately 2:30p.m. to 4:00p.m. Twelve children, ages twenty-two months through five years old, were in care, for a total of twenty-four points, with two caregivers. Caregivers and children were observed in the indoor space engaged in activities to arising from nap time, snack time and free play. Interviews and interactions were conducted and no medications are given at this time. Five children?s records, three caregiver records and one household member's record were reviewed. The Provider was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left. Standard #: 22VAC40-111-920
Description: Based on observation, the licensee failed to date a menu when meals or snacks are provided by the family day home. Evidence: 1. There was no current menu posted for the week. 2. Caregiver #1 acknowledged the menu was not posted.

Plan of Correction: Menu will be posted every Sunday night.

Standard #: 22VAC40-111-570-A
Description: Based on observation, the licensee failed to ensure that a caregiver does not exceed 16 points when determining if an additional caregiver is needed.
Evidence: 1. Upon entering the home, twelve children were observed in care, with one caregiver for approximately thirty minutes before an additional caregiver arrived. 2. Caregiver #1 reported the children in care included one five year old, two four year olds, four three year olds, four two year olds, and one twenty-two month old child, totaling twenty-four points. 3. Caregiver #1 acknowledged an additional caregiver was needed during the time she was alone with the children and advised Caregiver #2 had left for an appointment.

Plan of Correction: One assistant now lives in the home. All appointments will be scheduled when two people are able to be present.
