63.2 Licensure and Registration Procedures

An unannounced monitoring inspection was conducted on 8/6/19 from 10:30am until 11:20am. At the time of entrance, there were 4 children in care with the provider, for a total of 10 points. A sample of 4 children's records and 1 provider record were reviewed. Children were observed going for a walk with the provider, eating snack and watching an educational program on television. First aid and emergency supplies, documentation of emergency practice drills, the emergency preparedness plan and family day home postings were reviewed. The provider stated there have been no injuries to children in care since the last inspection. One violation in the area of transportation is identified on the violation notice and was discussed with the provider during the exit interview.

Standard #: 22VAC40-111-980-A
Description: Based on interview and a review of 4 children's records, written permission was not obtained from the parent of each child for the provider to take the child off the premises of the family day home. Evidence: 1. During the inspection, the provider stated that she takes the children on field trips, including walks around the neighborhood. 2. The records for child 1, child 2, child 3 and child 4 do not contain a general written permission from the parent to take the child off the premises. 3. The provider confirmed that the records for the children do not contain written permission from the parents to take the children on field trips.

Plan of Correction: The provider stated she has previously had the parents sign permission forms for her to take the children on field trips but could not locate the forms during the inspection. The provider will have all parents sign new permission forms today at pick up.
