Comments:
An unannounced monitoring inspection was conducted today from 11:15am-1:20pm. There were 8 children (20 points) directly supervised by 3 staff. The home, 3 staff records, 5 children?s records, 4 household member?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing freely, having water play outdoors, and eating lunch. Today's lunch consisted of spaghetti with meatballs, tomato sauce, mixed vegetables, and milk. Diapering procedures were also observed. There was an abundant supply of books, toys, and materials for the children. Areas of non-compliance are identified on the Violation Notice. Standard #: 22VAC40-111-60-B
Description: Based on record review, the family day home did not maintain complete children's records. Evidence: Child#1's record did not contain a complete work address for both parents. Child#5's record did not contain the father's work address.

Plan of Correction: I will ask the parents to provide the missing information for Child#1. For Child#5 there isn't a work address because their parent has quit their job.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation, the findings of the most recent inspection of the facility were not posted. Evidence: The Violation Notice issued on 02/27/2019 was not posted. The bulletin board area where it usually hangs did not have it in place.

Plan of Correction: I will post it.
