Technical Assistance:
The provider will need to show evidence that a central registry background check has been submitted for household members #2 and #3 within the next10 business days to licensing inspector. The next anticipated inspection will be the provider?s renewal inspection. Provider was reminded to have all renewal application documents completed and sent into licensing office 45 days prior to expiration for processing. The renewal application is available on the VDSS website.

Comments:
Today an unannounced Monitoring inspection took place between 10:10am and 1:20pm. There were 11 children present with the provider and 1 assistant for a total of 24 points within the supervision guidelines. The children were observed using play dough, reading books, playing ring-around-the-rosie and eating lunch (chicken nuggets, french fries, cucumbers, strawberries and milk). A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Violations:
Standard #: 22VAC40-111-800-B
Description: Based on record review, the provider did not review the emergency plan at least annually. Evidence: The last documented review of the provider's emergency is dated 5/1/2018.

Plan of Correction: This was corrected during the inspection.

Standard #: 22VAC40-111-60-B
Description: Based on record review, children's records did not contain all required information. Evidence: Child #1's file did not contain documentation of the review of the child's emergency contact information. Child #9's file did not contain documentation of the families medical insurance policy number.

Plan of Correction: I'll let the parents know so that they can bring my the copy. Medical insurance number was filled in during inspection.

Standard #: 22VAC40-111-80-B
Description: Based on record review, the provider did not verify the identity and age of a child within the first seven days of attendance. Evidence: Child #3's (first date of attendance: 5/24/19) file did not contain documentation that the child's age and identity has been reviewed.

Plan of Correction: I'll have the parents bring it in the next time he comes in.

Standard #: 22VAC40-111-250-A
Description: Based on observations, hanging items such as ropes were not out of reach of children under the age of five. Evidence: A rope was observed attached to a window latch and looped around a nearby door hand to keep the door open. The looped rope is a potential strangulation hazard. A group of toddler aged children were observed playing in this room.

Plan of Correction: This was corrected during the inspection.

Standard #: 22VAC40-111-500-F
Description: Based on observation, trampolines were used during the hours children were in care. Evidence: 8 out of 11 children were observed taking turns jumping on the provider's small indoor trampoline.

Plan of Correction: It will not come out again.

Standard #: 22VAC40-191-40-D-5-a
Description: Based on record review and interview, the provider has not obtained a central registry background check for each household member over the age of 14 years old. Evidence: The provider confirmed that household member #2 is 14 years old and household member #3 is 15 years old. There are no central registry results for either household member.

Plan of Correction: I might be able to do this today.
