Discussed that when cots are being used for sleeping or napping by children, the cots must have linens consisting of a top cover and a bottom cover or a one-piece covering that is open on three edges.

An unannounced renewal visit was conducted today between the hours of 8:40am through 10:30am with the provider on site. There were 6 children (9 points) with the provider. A sample of 6 children's files and 4 staff/household member's files were reviewed. The children were engaged in the following: toy play, playing with playdough, listening to music and coloring. Interactions between the children and caregiver were positive. The physical space, emergency supplies, evacuation drills, attendance records and the Emergency Preparedness Plan were reviewed. Standard #: 22VAC40-111-480-D Description: Based on observation, equipment used by children were not maintained in accordance with the manufacturer's instructions. Evidence: 1. The slide attached to the swing set was missing a bolt at the top of the slide and 2 bolts were rusting. Plan of Correction: The bolts were replaced during the inspection.
Standard #: 22VAC40-111-500-D Description: Based on observation, outdoor play equipment did not meet all requirements. Evidence: 1. Two upper S hooks on the baby swing were not tightly closed. Plan of Correction: I will get them closed today.
