Comments:
An unannounced monitoring inspection was conducted with the provider on July 31, 2019 beginning at 9:00 AM and ending at 10:10 AM. There were four children in the care of the provider during the inspection. The children's ages were 19 months, 19 months, 22 months, and 2 years old for a total of 11 points. The children were observed finishing morning snack which consisted of goldfish cracker, dry cereal, banana slices, and water. The children were also observed playing with balls, dancing, playing with trains and instruments. Four children's records were reviewed. Three household records were reviewed and in compliance. There were no medications on site to be reviewed. The emergency escape drill log was reviewed and in compliance. The most recent emergency evacuation drill was documented as 7/17/19. The most recent shelter-in-place drill was documented as 6/4/19. The first aid kit, flashlight, and weather band radio were reviewed and in compliance. Standard #: 22VAC40-111-100-A
Description: Based on a review of four children's records, the provider did not obtain documentation of a physical examination within 30 days after the first day of attendance for all children enrolled. Evidence: Child A, start date 6/11/19 did not have documentation of a physical examination in their record.

Plan of Correction: The provider has spoken with the parent regarding the missing documentation and the parent is working on obtaining the missing documentation to give to the provider. The provider will remind the parent that she needs the missing documentation for the record.

Standard #: 22VAC40-111-60-B
Description: Based on a review of four children's records, the provider did not have complete records for all the children enrolled. Evidence: Child A's record was missing the proof of the child's age and identity, policy number of the child's medical insurance, documentation of immunizations, and results of the health examination for the child.

Plan of Correction: The provider will request the missing information and documentation from the parents.

Standard #: 22VAC40-111-90-A
Description: Based on a review of four children's records, the provider did not obtain documentation that all children enrolled were adequately immunized prior to their first day of attendance according to the requirements of ? 32.1-46 A of the Code of Virginia. Evidence: Child A, start date 6/11/19, did not have documentation of immunizations in their record.

Plan of Correction: The provider has spoken with the parent regarding the missing documentation and the parent is working on obtaining the missing documentation to give to the provider. The provider will remind the parent that she needs the missing documentation for the record.
