An unannounced monitoring inspection was conducted on July 31, 2019 from approximately 9:55 am to 1:00 pm. Provider and assistant were present with eleven children for a total of twenty-four points. Five children's records were reviewed. Provider and household member records were also reviewed. Please complete the columns for ?description of action to be taken? and ?date to be corrected? for each violation cited on the violation notice, and then return a signed copy to the licensing office within 5 calendar days of receipt. If you have any questions, contact your licensing inspector at (757) 404-5261. The providers responses for the ?Description of Action To Be Taken? were not received as of August 13, 2019 and will not appear on this ?Violation Notice?.
Standard #: 22VAC40-111-920 Description: Based on observation, provider did not ensure when meals or snacks are provided by the family day home, the menu for the current week is posted. Evidence: The home is licensed for nine children. There were eleven children present in care at the family day home. Provider stated two children are attending as a "trial" for a few hours two days this week and two days next week with the plan to start attending full-time in September.
