Standard #: 22VAC40-111-750-B Description: Based on observation of the family day home it was determined that not all topical skin products were kept inaccessible to children. Evidence: A container of lotion, aquaphor, cocoa-butter, and antibiotic ointment were observed under the desk area in the day care area. Plan of Correction: Removed during inspection.
Standard #: 22VAC40-111-870-G Description: Based on observation and interview with the provider it was determined that tables were not cleaned after each use. Evidence: Upon arrival at the family day home at 1:30 p.m. the LI (Licensing Inspector) observed various food plates, sippy cups, and food debris on the feeding table and floor. The table was not cleaned after the children finished lunch. Plan of Correction: Spoke with helper that ha to be a priority above all.
Standard #: 22VAC40-111-50-A Description: Based on observation and interview with the caregiver it was determined that a written record off attendance was not kept daily. Evidence: There was no written record of attendance observed during the inspection. Plan of Correction: Provider and staff will sign kids in ourselves and not depend on parent to sign in.
Standard #: 22VAC40-111-180-A Description: Based on review of the provider's record it was determined that not all TB (tuberculosis) screenings were repeated every two years. Evidence: The most recent TB screening for the provider is dated 06/02/2017. Plan of Correction: Will obtain a TB screening.
Standard #: 22VAC40-111-280 Description: Based on observation of the family day home it was determined that not all potentially poisonous materials were inaccessible to children. Evidence: A container of medicated topical ointment and a container of cleaning wipes were observed on the floor of the desk area. Both items state to keep out of reach of children. Plan of Correction: Removed during inspection.
Standard #: 22VAC40-111-410 Description: Based on observation of the family day home it was determined that not all portable electric fans were securely mounted and kept out of reach of children. Evidence: A tower fan was placed on top of a toy kitchen set. The fan was not mounted and accessible to children. Plan of Correction: Removed during inspection
Standard #: 22VAC40-111-560-A Description: Based on observation and interview with the provider and caregiver it was determined that direct supervision was not provided at least every 15 minutes for sleeping infants. Evidence: Four children and one infant were observed sleeping in a nap room with the door closed. The caregiver was observed sitting with another child in the day care area which is adjacent to the nap room. It was reported by the provider and caregiver that in-person observations of sleeping infants are conducted every 20 to 30 minutes. Plan of Correction: Have set alarm to go off every 15 minutes as a reminder.
