Comments:
An unannounced monitoring inspection was conducted on August 6, 2019 from approximately 12:00 pm to 1:00 pm. Provider was present with two children for a total of four points. Two children's records were reviewed. Please complete the columns for ?description of action to be taken? and ?date to be corrected? for each violation cited on the violation notice, and then return a signed copy to the licensing office within 5 calendar days of receipt. Violations:
Standard #: 22VAC40-111-920
Description: Based on interview, provider did not ensure when meals or snacks are provided by the family day home, the menu for the current one-week period shall be posted. Evidence: Provider stated she did not have the menu for the current week posted.

Standard #: 22VAC40-111-50-A
Description: Based on observation, review and interview, provider did not ensure there is a written record of children in attendance each day. Evidence: Between 12:00 and 12:30 pm, two children were observed in care. The attendance sheet had one child documented as being present. Provider stated the child not noted on the attendance sheet had arrived at 11:00 am.

Standard #: 22VAC40-111-60-B
Description: Based on review, provider did not ensure 2 of 2 children's records have documentation of required information. Evidence: The following was found during record review: 1. The record for child #1 did not have first date of attendance, father's employment information (place, address and phone number), mother's home address, insurance information (plan and number) and address for the first emergency contact. 2. The record for child #2 did not have first date of attendance, father's home address, mother's employment phone number, insurance plan number, two emergency contact addresses and two emergency contact phone numbers.
