Comments:
An unannounced monitoring inspection was conducted today from 10:45am-1:00pm. There were 8 children (16 points) supervised by 2 direct care staff. The home, 2 staff records, 4 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were coloring, playing with cars and trains, watching an educational video, dancing, and eating lunch. Lunch served was turkey, crackers, broccoli, bananas, and milk . There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Description: Based on record review, 1 of 4 children's records did not contain documentation of all required information. Evidence: Child #2's record did not contain documentation of parental written authorization if a caregiver is to administer prescription or nonprescription medication to the child as required by 22 VAC 40-111-700 A 2.

Plan of Correction: I will have the parent sign today.

Standard #: 22VAC40-111-240-A
Description: Based on observation, all areas and furnishings of the family day home were not maintained in a clean, safe, and operable condition. Evidence: There was an area of chipped and broken drywall and peeling paint on the wall next to the rocking chair.

Plan of Correction: My husband will paint the area.

Standard #: 22VAC40-111-280
Description: Based on observation, the provider did not ensure that potentially poisonous substances were stored away from food in areas inaccessible to children. Evidence: A bottle of hand sanitizer was sitting on top of the diaper changing table.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-111-320-B
Description: Based on observation, the provider did not ensure that all unused electrical outlets and surge protectors accessible to children under five years of age had child-resistant protective covers. Evidence: 1. The naptime room had 2 electrical outlets behind an infant crib that were not covered. 2. The children's bathroom had 1 electrical outlet next to the sink that was not covered.
