63.2 Licensure and Registration Procedures

Technical Assistance:
Technical Assistance was offered regarding the following: Background checks include forms and time lines, cot placement for nap time and staff records.

Comments:
An unannounced monitoring inspection was conducted from 10:45am to 12:45pm. There were 11 children (27 points) in direct care and supervision of 3 caregivers. The children were observed washing hands for lunch and eating lunch. The lunch menu included tuna, tortilla bread, green beans and milk. The children were also observed napping. 5 children's records, 2 household member records and 2 caregiver records were reviewed. Additional documents reviewed included the emergency drill records and plan. Areas of non compliance are listed in the violation notice. Violations:
Standard #: 22VAC40-111-100-A
Description: Based on record review, the provider did not obtain documentation of a physical examination for a child enrolled, within 30 days of enrollment. Evidence: Child # 1's date of enrollment was 8/29/16. Child # 1's record did not have any documentation of a physical exam. It was not obtained within 30 days of enrollment.

Plan of Correction: I will ask the parents to submit the physical.

Standard #: 22VAC40-111-120-A
Description: Based on record review, the provider did not maintain a complete record for a caregiver. Evidence: Caregiver # 1's record was missing the first date of employment, and information for an emergency contact.

Plan of Correction: I will complete the record today.

Standard #: 63.2-1720.1-B-3
Description: Based on record review and staff interview, the family day home did not obtain out of state central registry findings for a caregiver that lived outside of Virginia in the last five years. Evidence: 1. Caregiver # 1 stated that she began employment on 5/28/19. Caregiver # 1 stated that she moved from Maryland to Virginia in 2015. 2. Caregiver # 1's record had documentation that showed that she had maintained residency in Maryland. 3. Caregiver # 2 stated that she did not know that out of state central registry findings had to be obtained. Caregiver # 1's central registry findings for Maryland were not obtained within 45 days of employment.

Plan of Correction: I will submit the form as soon as possible.

Standard #: 63.2-1721.1-B-2
Description: Based on record review, the provider did not obtain fingerprint results for an adult household member upon reaching the age of 18 years. Evidence: 1. Household Member (HHM) # 1 turned 18 years of age in May 2019. 2. Fingerprint results were not obtained for HHM #1 upon reaching the age of 18 years.

Plan of Correction: I will make the appointment today and get this done.
