Comments:
An unannounced renewal inspection was conducted today between the hours of 9:25am through 11:45am with the provider on site. There were 3 children (8 points) with the provider and assistant. A sample of 3 children's files and 5 staff/household member's files were reviewed. The children were engaged in the following: toy play, outside time, snack and hand washing. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. Interactions between the children and staff were positive. The physical space, emergency supplies, evacuation drills, attendance records and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-111-730-C
Description: Based on observation, when medication is stored in a refrigerator used for food, the medications were not stored together in a container or in a clearly defined area away from food. Evidence: 1. The provider had 2 medications in the refrigerator on the door shelf with other food and beverages and was not stored in a container or away from food.

Plan of Correction: I will put it in a container upstairs.

Standard #: 22VAC40-111-730-D
Description: Based on observation, medications were not kept in a locked place using a safe locking method that prevent access by children. Evidence: 1. Two packets of tylenol were inside the first aid kit. The first aid kit was kept on a shelf in the room the children are cared for and was not kept locked.

Plan of Correction: I removed them immediately.

Standard #: 22VAC40-111-90-C
Description: Based on review, 2 of 3 children's records did not have documentation of additional immunizations every six months for children under the age of two years. Evidence: 1. Child #1 (child under the age of 2 at the time of enrollment) had an immunization record dated 8/01/2018 and did not have an updated record every 6 months in the file. 2. Child #2 (child under the age of 2 at the time of enrollment) had an immunization record dated 3/29/2016 and did not have an updated record every 6 months in the file.

Plan of Correction: I will ask the parents for the update today.

Standard #: 22VAC40-111-210-A
Description: Based on interview, the caregivers did not obtain 16 hours of training annually. Evidence: 1. The caregiver stated that caregiver #1 and caregiver #2 had not completed 16 hours of training annually.

Plan of Correction: I will make sure the training is completed.

Standard #: 22VAC40-111-240-A
Description: Based on observation, playground equipment of the family day home outside were not maintained in a safe and operable condition. Evidence: 1. The swing was missing a bolt.

Plan of Correction: I will have it fixed today.

Standard #: 22VAC40-111-320-A
Description: Based on observation, the small unused electrical appliances were not unplugged when not being used. Evidence: 1. A toaster oven in the upstairs kitchen was plugged in and not being used.

Plan of Correction: I unplugged it during the inspection.

Standard #: 22VAC40-111-500-C
Description: Based on observation, the stationary outdoor playground equipment did not meet all requirements. Evidence: 1. The swing set is placed approximately 2 1/2 feet away from the fence on one side and approximately 2 feet 10 inches away from the fence on the other side and is not at least six feet from the perimeter of the fence.

Plan of Correction: I will take it down.
