Comments:
An unannounced renewal inspection was conducted today, August 21, 2019, from 10:00am-11:15am. There were 4 children (11 points) plus the provider's own child directly supervised by 1 staff. The home, 1 staff record, 3 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, building with blocks, and an infant was observed having play time. There was a supply of books, toys, and materials for the children. Standard #: 22VAC40-111-760-A-1
Description: Based on observation, the provider did not maintain all required items in the first aid kit. Evidence: The first aid kit is missing one triangular bandage.

Plan of Correction: I will get a new second triangular bandage.

Standard #: 22VAC40-111-100-A
Description: Based on record review, the provider did not obtain documentation of a physical examination within 30 days after the first day of attendance. Evidence: Child 1 and 2 do not have documentation of a physical examination within 30 days after the first day of attendance of 07/17/2019 .

Description: Based on record review, the provider did not maintain complete records for each child. Evidence: Child 1 and 2 are missing first date of attendance, one parent work phone number, doctor phone number and street address, medical insurance policy, and the street addresses of two emergency contacts.

Description: Based on record review, the provider did not obtain documentation that all children are adequately immunized prior to attendance in the family day home.
