A unannounced renewal inspection was conducted with the provider and one assistant. There were eight children in care ranging from 12 months to eight years old. Children were observed during group time, lunch and rest time. Two staff files and one household files were reviewed along with six children's files. A whole home walk thru was conducted with the provider. All requied paper work was posted was posted. The inspection time was 11:30am -2:00pm.

Standard #: 22VAC40-111-120-A Description: Based review of the two caregivers records not all were complete. Evidence Caregiver A with a start date of 5/28/18 did not have documentation of two references and did not have documentaton of orientation. Plan of Correction: I will check references and complete orientation.
Standard #: 22VAC40-111-120-D Description: Based on the review of the annual training not all caregivers attended 16 hours of annual training. Evidence: Staff A had documentation of eight hours of annual training. Plan of Correction: We will make up the missing training hours and complete the 16 hours.
Standard #: 22VAC40-111-60-B Description: Based on review of Six children's records not all were complete. Evidence: Child A with a start date of 7/1/18 was missing The insurance policy number, Child B with a start date of 5/1/17 was missing one emergency contact. Plan of Correction: I will ask the parents for the missing information.
Standard #: 22VAC40-111-320-A Description: Based on observation not all small appliances were not unplugged when not in use. Evidence: the toaster. coffee pot and blender were all plugged in when not in use posing a fire hazard Plan of Correction: I will keep all small appliance unplgged.
Standard #: 22VAC40-111-320-B Description: Based on observation not all out lets had protective covers. Evidence: one out let in the infant room and two out lets in the day care area did not have protective cover. Plan of Correction: Corrected during the inspection.
Standard #: 22VAC40-665-390-B-2 Description: Based on observation not all emergency supplies worked during the inspection. Evidence: The provider had a battery operated radio however it did not work during the inspection. Plan of Correction: I will buy another weather band radio.
Standard #: 22VAC40-665-410-A-2 Description: Based on review of the shelter and place log the provider did not ensure there were two shelter in place drills conducted during the past year. Evidence: there was no documentation of two shelter in place drill being conducted. Plan of Correction: I will conducted and document all drills.
