An unannounced mandated Monitoring inspection was conducted today 8/21/19 from 2:00 pm to 4:00pm. There were 7 children (21 points) with 2 caregivers. The Provider was not on site during this inspection. Children were observed sleeping, having snack (bananas, veggie sticks, milk), dancing, and playing with toys. Reviewed 2 staff files, 3 children's files, menu, attendance, crib compliance, emergency evacuation log, and physical plant. Violations of the Standards are recorded on the Violation Notice. Standard #: 22VAC40-111-60-B Description: Based on record review, children's record were incomplete. Evidence: 1. Child#2 was missing documentation of the annual review of the child's emergency contact information. Last update was 7/26/18. Plan of Correction: I asked the parent and sent you a copy
Standard #: 22VAC40-111-70-B Description: Based on record review, the provider did not obtain written parent's acknowledgement regarding the policies of the facility. Evidence: Child#1 (start date 8/19/19) did not have a parent's signature acknowledging the status of whether or not there is liability insurance in the family child care home. Plan of Correction: I asked the parent and sent you a signed copy
Standard #: 22VAC40-111-480-B Description: Based on observation and interview, the equipment and materials used by a child were not appropriate to the age, size, ability, and interest of the child. Evidence: Child#4 (35 months) was observed sitting in a high-chair for snack. Once removed, the child was observed sitting and eating snack appropriately with 2 other children at a table. Plan of Correction: Child was removed from the high-chair and placed at an age appropriate table and chair.
Standard #: 22VAC40-191-60-C-2 Description: Based on record review, provider did not obtain a central registry finding within 30 days of employment. Evidence: Staff#1 (start date 1/8/19) did not have documentation of a central registry finding. Plan of Correction: I had her re-do the Central Registry. I called and they could not find it so I did another one.
