Comments:
An unannounced monitoring inspection was conducted today from 10:00am-11:15am. There were 11 children (32 points) directly supervised by 3 staff. The home, 1 staff records, 3 children?s records, 1 household member?s record, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, playing outside, building with blocks and riding toys. Infants were observed being held during bottle feeding. There was a supply of books, toys, and materials for the children. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-280
Description: Based on observation, poisonous substances were not stored in a area inaccessible to children. Evidence: In the bathroom utilized by children, mouth wash and rubbing alcohol was observed in a unlocked medicine cabinet.

Plan of Correction: The items were removed at the time of inspection.

Standard #: 22VAC40-111-520-B
Description: Based on observation, the crib was used as a place space for infants. Evidence: Child 1 was observed in a crib with a playmat in it. Staff stated Child 1 was placed in the crib so they could have a safe place to play.

Plan of Correction: Child 1 was removed from the crib and placed in a portioned off section of the day care area to be safe and play on their stomach and the playmat.
