A modification study was conducted on Wednesday, August 21, 2019 from 1:48pm to 2:45pm due to a change in location. The inspector received a modification request on August 14, 2019. All areas of the home including rooms, bathrooms, hallways, kitchen, closets and outside areas were inspected to ensure licensing standards are being met. During the modification inspection, the home was observed to be well laid out. The provider has submitted all of the required application materials at this time.
