A Subsidy Health and Safety inspection was conducted on August 27, 2019 to review supplemental health and safety requirements. Provider was present with four children for a total of seven points. Four children's records were reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: 22VAC40-665-150-G
Description: Based on review and interview, provider did not ensure there is a current written list of all children's allergies, sensitivities and dietary restrictions that is dated and kept confidential. Evidence: The record for child #3 had documentation of a dietary sensitivity/restriction. Provider stated she does not have a list with this information.

Plan of Correction: Will make a list and keep it in a folder in the child care room.

Standard #: 22VAC40-665-400-A
Description: Based on review, provider did not ensure the written emergency preparedness plan addresses all required components. Evidence: The emergency preparedness plan did not address the following: accommodations or special requirements for infants, toddlers and children with special needs during an evacuation and shelter-in-place, lockdown procedures, and continuity of operations procedures to ensure essential functions are maintained during an emergency.

Plan of Correction: Will correct the plan.
