Comments:
An unannounced monitoring inspection was conducted on Wednesday July 31, 2019 to determine compliance with licensing standards. The inspector arrived at 3:30 PM and concluded the inspection at 5:25 PM. Today, there were eight children, present with 2 caregivers. During the inspection, children were observed napping, washing hands, eating snack and creating an art project. Transportation is not provided. Children?s equipment, learning materials, indoor space and restrooms were inspected. Daily attendance records, fire drill, shelter in place and all required postings were reviewed. The provider was not available for the inspection. The inspector completed the exit interview with the caregivers present. An Acknowledgement of Inspection form was signed and left with the caregivers. Violations were cited and are documented on the violation notice. Standard #: 22VAC40-111-80-B
Description: Based on a review of children's records, the provider did not ensure that the identity and age of a child was verified by viewing a certified birth certificate, birth registration card or Notification of birth. Evidence: 1. The records for child #2 and child #5 did not have documentation of verification of identity and age. 2. Caregiver #3 was unable to locate documentation for child #2 and child #5.

Standard #: 22VAC40-111-460-C
Description: Based on a review of documentation, the provider did not ensure that household pets shall be vaccinated. Evidence: 1. The current shot record for Dog #1 shows the rabies vaccination expired on 06/03/2019. 2. The current shot record for Dog #2 showss the rabies vaccination expired on 06/03/2019.

Description: Based on observation and interview, the provider did not ensure that children's hands are washed with liquid soap and warm running water after toileting or diapering. Evidence: 1. Caregiver #2 did not wash the hands of the child she changed after diapering. 2. Caregiver #2 stated she did not wash the child's hands after the diaper change.

Description: Based on observation, the provider did not ensure that the diapering surface shall be nonabsorbent. Evidence: The diaper changing pad had a cotton changing pad cover.
