An unannounced monitoring inspection was conducted today with the provider, who was present with one assistant and 8 children for a total of 16 points. The children were on their cots napping during the inspection. For lunch, the provider reported that she fed the children spaghetti with meat balls, broccoli, tomato and milk. Six children and two assistant's files were reviewed today.

Standard #: 22VAC40-111-70-A Description: Based on a review of six children's files and an interview with the provider, it was determined that parents were not providing information in writing prior to their child's firs day of attendance.
Evidence:
The parents of child A, B and C were not provided in writing information regarding termination policy, general schedule, information on the use of assistants/substitute providers etc. Children A through C enrolled on 7/8/19.Plan of Correction: I will make sure to give the parents all of the information in standard 70.
Standard #: 22VAC40-111-200-A Description: Based on a review of the two assistant's files and an interview with the provider, it was determined that one assistant was not provided orientation training by the end of their first week of assuming job responsibilities.
Evidence:
Assistant A was hired on 8/12/19 and had not been given orientation training.Plan of Correction: I will give and document the orientation.
