Comments:
An unannounced monitoring inspection was completed on August 21, 2019 from approximately 10:45 a.m. to 12:00 p.m. Seven children were in care, for a total of ten points, with two caregivers. Caregivers and children were observed in the indoor and outdoor space engaged in activities to include outside play and preparation for lunch. Interviews and interactions were conducted and no medications are given. Five children?s records, four caregiver records and one household member record were reviewed. The Provider was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left. Violations:
Standard #: 22VAC40-111-750-A
Description: Based on an observation, review of children's records and interview, the licensee failed to obtain written parent authorization when topical skin care products such as insect repellent are used.
Evidence: 1. A can of insect repellent was observed outside where Caregiver #3 was supervising children playing. In an interview with Caregiver #1, she reported that the children are sprayed with insect repellent when they go outside at their parents' request. A review of children's record revealed no topical skin product authorizations have been documented for any child in care. 2. Caregiver #1 acknowledged she does not have written authorization for any child to use inspect repellent and reported being unaware of this requirement.

Plan of Correction: The provider will obtain written authorization, using the division's form provided online, from each child's parent to use insect repellent.

Standard #: 22VAC40-111-120-B
Description: Based on a review of caregiver records and interview, the licensee failed to ensure caregiver records contain all the required elements for one of four caregivers.
Evidence: 1. Caregiver #2's record, hired on July 25, 2017, lacked documentation of training as required for the job position. 2. Caregiver #1 reported that Caregiver #2 has completed the required annual training but that she has not submitted training documentation to include certificates.

Plan of Correction: The provider will obtain training documentation for Caregiver #2 for her record.

Standard #: 22VAC40-111-180-A
Description: Based on a review of caregiver records, the licensee failed to ensure that each caregiver and adult household member obtain a tuberculosis screening every two years for one of four caregivers.
Evidence: 1. Caregiver #1's record contained a tuberculosis screening/test dated June 1, 2016 and there was no subsequent screening/test in the record. 2. Caregiver #1 acknowledged a subsequent screening/test has not been completed as required.

Plan of Correction: The provider will schedule a subsequent screening/test for Caregiver #1.

Standard #: 22VAC40-111-210-A
Description: Based on a review of caregiver records and interview, the licensee failed to ensure a caregiver obtained 16 clock hours of annual training for one of four caregivers.
Evidence: 1. Caregiver #4's record, hired in February 2016, lacked documentation of 16 hours of annual training for the last training year. There were approximately 3 hours of training documented. 2.Caregiver #1 acknowledged that Caregiver #4 has not completed annual training as required.

Plan of Correction: The provider will begin to count Caregiver #4's college courses as annual training for the upcoming year and document this. If it's determined that Caregiver #4 is unable to obtain the required training, the provider will consider no longer using Caregiver #4 as an assistant.
