Comments:

An Unannounced Monitoring Inspection was conducted on 8/30/2019 from 12:20pm to 1:35pm. There were 6 children (8 points) directly supervised by 2 staff. The home, 1 caregiver's records, 6 child's records, 1 household member and two pet's records, evacuation drills, emergency supplies, and policies were inspected. There was one medication on site today. Children were observed playing freely inside the play room, washing hands for lunch and eating lunch. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified in this report. Standard #:	22VAC40-111-920
Description:	Based on review, the provider did not ensure that the menu for the one-week period was posted and any subsitutions were provided and made to the menu. Evidence: Reviewed the menu that said "pot roast, crackers, potato salad, cabbage" but the provider was to give the children lunchables pizza instead.
Plan of Correction:	The menu changes regularly, but will try to update it.
Standard #:	22VAC40-111-240-A
Description:	Based on observation, the provider did not ensure that areas and furnishings of the family day home, inside and outside, shall be maintained in a clean, safe and operable condition. Evidence: Observed in the backyard: 1) Large pieces of wood leftover from fence/gate work 2) A pile of large stones leftover from yard work 3) A wooden pallet that was missing several planks leaning up against the side deck.
Plan of Correction:	Will be moved over the weekend.

Description:	Based on review the provider did not ensure that the fireplace was inspected annually by a knowledgeable inspector. Documentation of the inspection and cleaning shall be maintained by the provider. Evidence: Reviewed the provider's records and identified through the provider's statements that the provider has not completed a fireplace inspection within the last 12 months.
Plan of Correction:	Will obtain a fireplace inspection.
Standard #:	22VAC40-111-460-J
Description:	Based on observation, the provider did not ensure that animal excrement was removed promptly, disposed of properly. Evidence: Observed 2 piles of dog excrement, one of the piles appears to have been partially dissolved by rain and time.
Plan of Correction:	The backyard is usually cleaned up everyday before the children go outside.
