Technical Assistance:
Provided consultation on posting and dating the weekly menu.

An unannounced monitoring inspection was conducted from 12:00 - 1:15pm with the provider. There were five children in care, ranging from 16 months to four years of age, for a total of 13 points. The children were observed eating lunch, washing hands, having free play and watching an age appropriate television program/video. Lunch served today was chicken nuggets, whole wheat bread, green beans and apples with milk. Five children's records were reviewed and in compliance. Three caregiver/household member records were reviewed. The battery operated flashlight and weather band radio were observed and in compliance. The first aid kit was observed and in compliance. The last fire drill was conducted: 8/5/19 and the last shelter-in-place drill was conducted: 2/6/19. The provider and her assistant both have 17 hours of annual training. Standard #: 22VAC40-111-180-A
Description: Based on a review of three provider/household member files, it was determined that repeat tuberculosis (TB) test/screenings were not completed every two years. Evidence: The TB test for provider #1is dated: 4/21/17 and for household member #1 is dated: 4/26/17.

Plan of Correction: We got repeat TB tests and will ask the doctor for the documentation to put in the files.
