Comments:
An unannounced monitoring inspection was conducted between the hours of 9:00am through 11:15am. There were a total of 10 children present ages 3 months to 3 years (25 points) with the Provider and 2 assistants. A total of 5 children?s files, 3 staff files, and 2 household member?s files were reviewed. The children were engaged in the following: participating in circle time, singing, repeating nursery rhymes, learning days of the week in Spanish, practicing American Sign Language, and doing a coloring art project where they were making their own puzzles. Positive interactions between caregivers and children and nice transitions between activities were observed during this inspection. A complete inspection of the programming space, children and staff records, evacuation drills, menu, attendance, and emergency procedures were completed today. The provider is MAT (Medication Administration Training) certified, however, she advised there was no medication on site today. The provider is current in First Aid/CPR certification. There was an abundant supply of age appropriate materials and toys and the home was clean and organized. Please contact me with any questions or concerns at morgan.bryson1@dss.virginia.gov.

Standard #: 22VAC40-111-60-B
Description: Based on review of 5 children's records, 2 children's records did not contain all required information.

Evidence:
1. Child #2 (Date of Enrollment: 8/19/19) did not have the immunization record on file by the first day of attendance.
2. Child #5 (Date of Enrollment: 4/18/19) did not have a physical examination on file within 30 days of the first day of attendance or the immunization record by the first day of attendance.

Plan of Correction: I sent messages to both parents and I will give them until Friday to bring the documents in.

Standard #: 22VAC40-111-180-A
Description: Based on documentation review, the provider did not obtain for each adult household member a current Tuberculosis screening or test every two years.

Evidence:
1. Adult Household Member #1's last Tuberculosis test is dated 6/10/17.
2. Adult Household Member #2's last Tuberculosis test is dated 6/10/17.

Plan of Correction: We can get the TB screening today.
