Technical Assistance:
Written parental permission for the administration of topical ointments, such as diaper cream, insect repellent and sunscreen was discussed with the provider as the permission must include the actual brand name of the product to be applied.

Documentation of children's injuries that may occur at the family day home and those that occur at the child's home was discussed.

Written parental permission for photographs of the children to be shared with other parents and/or on social media was discussed.

Comments:
An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 11:30 AM and departed at 1:30 PM. Five children, totaling ten points, were in care with the provider. Transition from lunch to activities and then nap time were observed. Three children's records were reviewed. No changes in household members were reported. Health information for the new family cat was reviewed. The provider reported that no medication has been administered and no children's injuries were reported.

An exit meeting was conducted with the provider prior to closure of the inspection.

Violations:
Standard #: 22VAC40-111-830-A
Description: Based upon review of the emergency drill record, the provider has not ensured that emergency evacuation procedures are practiced monthly.
Evidence:
The evacuation drill record provided did not have an evacuation drill documented for August 2019.

Plan of Correction: The provider responded with the following:
Evacuation drills will be practiced and documented each month.

Standard #: 22VAC40-111-60-B
Description: Based upon review of three children's records and provider interview, the provider has not ensured that each child's record includes all required information.
Evidence:
1. The record provided for child 2 did not include the following:
- child's nickname, sex and address
- name, home address and telephone number of each parent who has custody
- names, addresses and telephone numbers of the places of employ for each person who has custody
- names, addresses and telephone numbers of two designated persons to be contacted in an emergency when a parent cannot be reached
-name, office address and telephone number of the child's physician
- information on allergies and intolerance to food
- name and policy number of the child's medical coverage
- names of persons other than custodial parents who are authorized to pick up the child
- chronic physical problems, pertinent developmental information and any special accommodations needed
- proof of the child's age and identity
and
- names of any previous child care or schools attended.
2. The provider confirmed that the above information was not available in the record for child 2..

Plan of Correction: The provider responded with the following:
The parent brought in all required paperwork for child 2, however it has been misplaced. Parent is already working on obtaining and completing the required information.

Standard #: 22VAC40-111-90-A
Description: Based upon review of three children's records and provider interview, the provider has not ensured that documentation that the child has been adequately immunized is obtained before a child may attend the family day home.
Evidence:
1. According to the record provided for child 2, child 2 began attending care on 08/20/2019. The record did not include documentation of immunizations for child 2.
2. The provider verified that the record for child 2 did not contain immunization documentation as the document had been misplaced.

Plan of Correction: The provider responded with the following:
The parent provided the documentation, however it has been misplaced. The parent has been notified and is replacing the missing documentation.
