Comments:
An unannounced monitoring inspection was completed on August 21, 2019 from approximately 12:45 p.m. to 2:30 p.m. Seven children, infants, were in care for a total of twenty-six points, with two caregivers. Caregivers and children were observed in the indoor space engaged in activities to include daily activities for infants. Interviews and interactions were conducted and no medications are given. Five children?s records and two caregiver records were reviewed. The Provider was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left. Violations:
Standard #: 22VAC40-111-780-B
Description: Based on a review of children?s records, the licensee failed to review with the parent annually the child?s emergency contact information to ensure the information is current for two of five children.
Evidence: 1. Child #1's record, enrolled on March 5, 2018 and Child #4's record, enrolled on, July 9, 2018, lacked documentation of review of the emergency contacts. 2. Caregiver #1 acknowledged these updates were not completed annually as required.

Plan of Correction: The provider will choose one annual date in which she will update all the children's emergency contacts at the same time each year. The provider will get a planner that includes a calendar and will begin tracking this date annually.

Standard #: 22VAC40-111-800-B
Description: Based on a review of the emergency preparedness plan, the licensee failed to review the plan at least annually and update the plan as needed.
Evidence: 1. The last recorded update of the emergency preparedness plan was observed and dated June 2017. 2. Caregiver #1 acknowledged the plan has not been reviewed annually as required.

Plan of Correction: The provider will get a planner that includes a calendar and will begin tracking this date annually.

Standard #: 22VAC40-111-830-C
Description: Based on review of records and interview, the licensee failed to maintain documentation of emergency evacuation drills.
Evidence: 1. The last documented fire drill was dated May 29, 2019. 2. In an interview, Caregiver #1 reported that fire drills have been completed monthly but have not been documented as required.

Plan of Correction: The provider will get a planner that includes a calendar and will begin tracking a reminder to document the fire drills monthly.

Standard #: 22VAC40-111-920
Description: Based on observation, the licensee failed to date a menu when meals or snacks are provided by the family day home.
Evidence: 1. A review of the week?s current posted menu lacked a date. 2. Caregiver #1 acknowledged the menu was not dated as required.

Plan of Correction: The provider will begin dating the menu.

Standard #: 22VAC40-111-960-E
Description: Based on observation, the licensee failed to ensure that bottles were labeled with the child?s name and date if more than one infant is in care.
Evidence: 1. There were five bottles observed in the refrigerator with no name or date labels. 2. Caregiver #1 reported that more than one child in care use bottles and acknowledged the bottles were not labeled with the date or child?s name as required.

Plan of Correction: The provider will order labels online today and begin using them when they are delivered.
