Comments:
An unannounced monitoring inspection was conducted from 10:30am through 12noon. There were 12 children in care ages 19 months through three years old. Points for children in care totaled 24. There were two caregivers present. The provider was not present at the onset of the inspection. By the conclusion of the inspection the provider was present in the home. Children were observed watching television, eating lunch, restroom routines, transitions and nap. Children and caregiver records were reviewed as well as required postings, emergency drills and emergency items. The provider stated that there is not any medication being administered to children in care. The provider stated that children in care are not transported by the family day home. Violations in the areas of administration, caregiver training, emergencies and nutrition were identified and appear on the violation notice. These violations were reviewed with the provider at the conclusion of the inspection.

Description: Based on review of first aid kit and interview the provider failed to maintain the kit with the required items.
Evidence:
1 - The first aid kit does not contain more than one triangular bandage.
2 - The provider stated that she was unable to locate an additional triangular bandage.

Description: Based on observation the provider failed to ensure that food was served in a clean and sanitary manner.
Evidence:
Child 1 was served a slice of bread on the bare table after the table had been wiped with a wet soapy cloth. The table remained wet from the cloth when bread was placed directly on the table. Child 1 ate the bread given to him.

Description: Based on observation the provider failed to ensure that meals serve follow the most recent, age-appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA).
Evidence:
1 - A grain was not served to children with the meal. Children were served meatballs, corn, green beans and milk.

Description: Based on observation and interview the provider failed to ensure that the total number of children receiving care at any one time does not exceed the maximum licensed capacity of the home.
Evidence:
1 - Upon entering the home at 10:30am there were 12 children sitting on the floor, watching TV, in the child care room with caregiver 2 and 3. There were not any other adults in the home at that time.
2 - Caregiver 2 and 3 confirmed that there were 12 children in care.

Description: Based on record review and interview the provider failed to ensure the caregivers obtain a minimum of 16 clock hours of training annually in areas relevant to their job responsibilities.
Evidence:
1 - The records of caregiver 2 and 3 did not contain documentation of 16 hours of annual training as required.
a. The record for caregiver 2 contains documentation of 12 hours of training. Caregiver 2 has a documented date of hire as May 11, 2018.
2 - The record for caregiver 3 contains documentation of 10 hours of training. Caregiver 3 has documented date of hire as October 6, 2015,
2 - The provider confirmed that there was not documentation of 16 hours of annual training in the record of caregivers 2 and 3.

Description: Based on observation and interview the provider failed to ensure that areas and furnishings of the family day home inside and outside are maintained in safe and operable condition.
Evidence:
1 - The outside deck has areas of rotting and deteriorating wood. The deteriorating wood was visible along the edge of the deck near and around the steps. Walking across the deck boards they were uneven, seemed to have areas of missing wood where shallow holes were forming.
a. Staff 1 stated that the children were taken outside to the deck for outside play.
2 - The backyard is littered with equipment, toys, bicycles, bar-b-q equipment

Standard #: 22VAC40-111-280
Description: Based on observation the provider failed to ensure that potentially poisonous substances, materials and supplies are stored in areas inaccessible to children.
Evidence:
1 - Upon entering the family day home there was a pump bottle of hand sanitizer sitting a table approximately 24 inches above the floor. Bottle states keep out of reach of children. The location of the hand sanitizer was an unlocked location and within reach of all the children in care.
2 - On the back deck, within reach of children are spacer compound, "ortho" house defense, flex color compound. All chemicals state hazardous keep out of reach of children.

Standard #: 22VAC40-111-500-C
Description: Based on observation the provider failed to ensure that stationary outdoor playground equipment is not installed over a hard surface.
Evidence:
1 - There are two climb and slide playground slides sitting on the bare deck on the back porch.
a. The provider stated that the deck is where the children have outside play daily.
