Comments:
An unannounced monitoring inspection was conducted September 9, 2019 to determine the providers compliance with family day home standards. The inspection was initiated at 10:30 am and concluded at 12:50 pm. Upon the inspectors arrival there were eight family day home children in care with two caregivers for a total of 20 points. Children were observed playing with toys and looking at books at the table. Diapering, bathroom breaks and hand washing was conducted. Later children had lunch that consisted of fish sticks, rice, mixed vegetables and milk. Nap time was observed with soft music being played in the background. All areas used for child care, inside and outside, were inspected. The family day home has age and stage appropriate materials. The first aid kit and non-medical supplies were inspected and found complete. The family day home had the most recent inspection and emergency numbers posted. The inspectors interacted with the caregivers and children in care.

Eight children's records, two caregiver records, and one household member's record were reviewed today.
One caregiver has current CPR and two caregivers have current first aid training. Medications are not administered nor does the provider offer transportation.
The most recent evacuation drill was documented on 08/28/2019. The two required shelter-in-place drill were completed on 02/27/2019 and 03/19/2019. A lock down drill was documented on 02/27/2019.

The violations cited during the licensing period were checked for corrections. There was one repeat violations cited during today's inspection. If you have questions, please contact the licensing inspector at 804-662-9758.

Violations:
Standard #: 22VAC40-111-780-B
Description: Based on review of eight children's records, the provider did not obtain the parent's signed acknowledgment of the annual review of the emergency contact information in the child's record for one child.

Evidence:

The most recent signed acknowledgement of review by the parent for the record of child #1 was dated 04/30/2018, exceeding the annual requirement.

Plan of Correction: Per the provider: I will have the parent review the record and sign tonight at pick up.

Standard #: 22VAC40-111-90-C
Description: Based on review of children's records, the provider did not obtain documentation of additional immunizations every six months for children under the age of two years.

Evidence:

The most recent documentation of immunizations for child #3, age 20 months, was dated 12/11/2018.

Plan of Correction: Per the provider: I will ask the parent to bring me a copy of the child's up dated immunizations for the record.

Standard #: 22VAC40-111-240-A
Description: Based on observation, the provider did not ensure all areas of the home, inside and outside, were maintained in a clean, safe and operable condition.

Evidence:

The wooden rocking chair in the main child care area is broken at the top of the seat back. Two spindles are not attached and extend out from the chair.

Plan of Correction: Per the provider: I will remove the chair from the area and have it repaired.

Standard #: 22VAC40-80-120-E-6
Description: Based on observation and interview, the provider did not post a copy of the special order issued by the department in a prominent place at each public entrance of the licensed premises to advise consumers of serious or persistent violations.

Evidence:

1. The Special Order issued by The Department and dated April 29, 2019 was not observed at the family day home.
2. The provider verified the Special Order was not posted.

Plan of Correction: The provider posted the Special Order on the main door of the family day home while the inspector was on site. Per the provider: I will keep the document on the door for parents to see.
