Technical Assistance:
Reviewed modification process once the provider gets Zoning approval to increase capacity to 12 children. All caregivers have until mid-October to complete the annual 16 hours of training.

Comments:
An unannounced renewal study was conducted today. Three children were in care for eight points with two caregivers. Observed children playing in the back yard, hand washing, snack and free play. Reviewed three caregiver records, three children records, emergency preparation, training hours, and physical space. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-111-90-C
Description: Based on record review, one of three children's immunizations records has not been updated every six months for a child under the age of two.
Evidence:
Child #3's last update was 12/2018.

Plan of Correction: Provider will contact the parent to get documentation of the current immunization record.
